NSI-189 phosphate is a nootropic compound best known for its ability to improve symptoms of major depressive disorder (MDD) and enhance overall cognitive functioning. NSI-189 was originally developed by the American pharmaceutical company, Neuralstem Incorporated, as a treatment for MDD. Additional research found that the compound was capable of acting as a treatment for cognitive dysfunction relating to Alzheimer’s, PTSD, brain injury, and age-related decline. Initial animal-based research conducted in July 2017 found that subjects showed significant improvement on secondary endpoints relating to depression and cognition. Further observation reported that there was little to no incidence of negative side effects related to administration of NSI-189.

A study conducted by Papakostas et. Al examined how efficiently NSI-189 is capable of treating MDD in animal test subjects. Over the course of 12 weeks the subjects were administered either a 40 mg dose of NSI-189, an 80 mg dose of NSI-189, or a placebo. Symptoms of depression were monitored and measured by various cognitive tests over the experimental period. Both the 40 mg and 80 mg doses were able to reduce the symptoms of depression in the subjects as well as improve cognitive test scores to a greater degree than the placebo group. However, the researchers reported that the 40 mg dose was more effective than the 80 mg dose (https://pubmed.ncbi.nlm.nih.gov/30626911/).

 

Cognitive Enhancement Through Use of NSI-189 Phosphate

The improvement in MDD was also associated with various pro-cognitive advancements. NSI-189 is believed to have neurogenic properties that promote neuron growth in the brain which results in many of the associated benefits of the compound. The mechanism of action for neurogenesis caused by NSI-189 is not entirely known, however, researchers have theorized the compound is able to modulate the activity of neural stem cells and progenitor cells. Both of these categories of cells are capable of transforming into specific neurons or neural precursors, leading to the promotion of neuron growth in damaged areas of the brain.

While the exact mechanism of action isn’t clear, researchers Naoki et. Al examined various ways NSI-189 may work to enhance neurogenesis in the brain. The initial steps of the study included inducing cerebral artery occlusion in Sprauge-Dawley rats. NSI-189 was then administered starting 6 hours after occlusion and every day after for a 12 week experimental period. First and foremost the study found that administration of NSI-189 led to a decrease in motor and neurological deficits caused by the cerebral occlusion. The research also reported that NSI-189 led to an increase in Ki67 protein expression and MAP2 density. When assessing the brains of the subjects it was noted that the immunoreactivity of MAP2 led to significant neurite outgrowth. Finally, the results reported that treating the animals with NSI-189 led to the reversal of cell death caused by oxygen-glucose deprivation (https://pubmed.ncbi.nlm.nih.gov/28181668/).

The research of Naoki et. Al also revealed that administration of NSI-189 led to upregulation of brain-derived neurotrophic factor (BDNF) and stem cell factor (SCF). These two compounds are heavily associated with the enhancement of cognitive functioning. BDNF plays a key role in plasticity in the brain, meaning that it is crucial to the improvement of learning and memory. BDNF plays multiple roles throughout the central nervous system. The compound binds to high affinity receptors such as TrkB which leads to the activation of signal transductions. Additionally, BDNF is crucial to beta cell survival and CREB and CRP production while also assisting in the regulation of glucose and energy metabolism. Decreased levels of BDNF are frequently seen in cases of Alzheimer’s Disease, Parkinson’s Disease, and various other neurodegenerative diseases.
